As a partner in your health care, it is important that you prepare for your appointment and bring the items necessary to make your appointment successful.

Plan to arrive at least 15 minutes early for your appointment to allow time for our registration process and history form completion. We make every effort to respect your time by staying on schedule.

Attire

It is suggested that you wear comfortable loose fitting clothing to your exam. Keep in mind that some examinations may require you to change into a gown. Please bring or wear shorts if you are coming for a hip or knee exam. For our female patients receiving shoulder exams, a tank top or sports bra is suggested.

What should I bring

  • Medical Records - If your visit is for a second opinion or on a physician's referral, you may need to bring medical records, x-rays, test results or other materials. Please bring these with you or arrange for them to be sent prior to your visit.
  • Personal Medical History - In order to provide the best care, your medical provider needs specific information about the medications you are taking and the details of your current injury. Please bring in a record of all current prescription and non prescription medications. Specific information about your current injury and any related history is also very helpful.
  • A completed Doctor Evaluation Form, if one is required for your visit. You can get the proper evaluation form by clicking here.
  • Insurance Information - As a courtesy to you, we are happy to bill your insurance carrier for you. In order to do this, we need a copy of your current insurance card. A $200.00 deposit is required at check-in for new patients who are unable to provide proof of insurance. If your insurance carrier requires a referral form, make sure it is obtained before your scheduled appointment.
  • Worker's Compensation Information (if applicable) - In order to file your worker's compensation claim, we need the full name and mailing address of your worker's compensation carrier, the date of the injury and the address and phone number of your employer at the time of the injury. In addition, we need your private healthcare insurance information. (see above)
  • Payment - Please be prepared at the time of service to pay the office visit co-pay required by your insurance plan. Some services may not be covered by insurance and would require payment at the time of your visit. We accept cash, checks and credit/debit cards. (Visa and MasterCard only)
